Making a New Accommodation Request

  • Submit the ADA Employee Reasonable Accommodation Request Form.
  • Ask a qualified health care provider or previous accommodation provider to either submit the ADA Employee Accommodation Verification Form and/or submit their own report documenting the conditions/diagnoses, functional limitations, and recommended accommodations (to be completed by Psychiatrist/Psychologist/Diagnosing Physician/Licensed Health Care Provider). Employees need to provide a copy of their job description to the health care provider for identifying their needs. Please contact access@utulsa.edu if you need to request a copy of your job description.
  • University Compliance Accessibility staff will then contact you.
