Making a New Accommodation Request
- Submit the Employee Accommodation Request Form.
- Ask a qualified health care provider or previous accommodation provider to either submit the Employee Accommodation Verification Form and/or submit their own report documenting the conditions/diagnoses, functional limitations, and recommended accommodations (to be completed by Psychiatrist/Psychologist/Diagnosing Physician/Licensed Health Care Provider). Employees need to provide a copy of their job description to the health care provider for identifying their needs. Please contact access@utulsa.edu if you need to request a copy of your job description.
- University Compliance Accessibility staff will then contact you.
Requesting a Change/Modification to Existing Accommodations
- Submit the Employee Accommodation Request Form.
